公证申请书的结构一般由六部分组成：标题、呈送机构、首部、正文、尾部、附件。

（一）标题

公证申请书的标题一般有以下两种写法。

1.单一性标题

单一性标题只写文种名称，如“公证申请书”.注意不能写成“申请书”.

2.两要素标题

两要素标题通常由公证对象和文种组成，如“技术合同公证申请书”.

（二）呈送机构

呈送机构即提交公证申请书的机构名称。

（三）首部

公证申请书的首部要写明以下两个内容。

1申请人的面然情况

（1）申请人如系个人，应写明姓名、性别、年龄、职业、籍贯、单位、住址、电话等内容。

（2）若委托代理人代为公证的，还应写明代理人的上述自然情况。

（3）申请人如果是法人的，应写明单位名称、法定地址、企业性质、开户银行、账号；法定代表人的姓名、性别、年龄、职务、籍贯、电话和住址。

2申请事项及作用

在这一项中主要应写明办理的公证是属于哪种公证。如证明经济合同的证明文件的副本与原本相符等。如果办理的公证有特殊用途，也应一并注明。

（四）正文

申请公证的正文应写明以下内容：

（1）请求公证的理由。

（2）具体事实情况。

（3）办理公证所要解决的问题。

（五）尾部

（1）写明申请人或单位，并签字盖章。

（2）法定代表人签字或盖章。

（3）注明申请时间。

（六）附件

附件即所提供的证明材料。应逐一说明办理公证所需的证明材料的名称、份数及有关证人的姓名、住址，如法人代表证明、委托书等。
