附注: 1.本表一式三份,在审批权限内报相关负责人签字(盖章),一份学生本人保存,一份由审批权限规定的部门保存,一份交教学系作为学生请假获批及任课教师考勤依据。

2.学生请假必须提交的相关材料：

① 因事请假,需本人提交书面申请,并说明请假理由、前往地点、联系人及联系方式、往返交通安排等； ② 因病请假,需提交县级(二甲)以上医疗机构出具的休假证明原件一份；

③ 因公请假,需提交用人部门出具的请假证明原件一份；

④ 若获假后需离校的学生,报批时必须同时提交请假期间家长签订的《安全责任承诺书》原件或传真件。

3.学生请假期间遇休息日或节假日，请假天数连续计算。

4.学生请假期满返校后,必须及时到批假人或单位销假。请假期满未及时销假者,按旷课处理。
